I/We understand that my child, as a participant in GEMS, Pioneer Boys, or Kids Quest, may be photographed or captured on video. I hereby grant full permission to Westminster Presbyterian Church to use photographs or videos - in print, electronic, online or other public media - of my child.
Permission to Dismiss Student without Parental Supervision
For the safety of your child, Westminster's Pick-up Policy for children birth through Grade 3 states: We (Westminster Church) ask that parents/guardians pick up their children within ten minutes following any class or program. Any parents/guardians desiring that their children be released independently or into the care of others must give written permission. Please list any other family members or adults who have your permission to pick up your child. If your child is in grade 4 and older, he/she will be dismissed without supervision at the end of the program.
